What is the best way to ventilate a cathedral ceiling?

On a true cathedral ceiling, which has an air space between the insulation and roof decking of at least 2 inches, a continuous soffit vent for intake and a continuous ridge vent for the exhaust is your best ventilation option.  This will ensure a continuous supply of air between every set of rafters.
